Job summary

Heritage Manor Care is seeking a dedicated LVN to join our clinical team in a skilled nursing setting in California. You will provide direct nursing care, supervise nursing assistants, and collaborate with physicians to ensure high-quality resident care.

Responsibilities include charting, medication administration, admissions/discharges, rounds with physicians, and assisting with care plans. A current California LVN license and background check clearance are required.

Qualifications

• Must hold a valid California LVN license.
• Ability to complete and file charts and physician orders.
• Experience administering medications under physician orders.
• Familiarity with care plan documentation and updates.

Responsibilities

• Provide direct nursing care to residents as assigned.
• Supervise day-to-day nursing activities by nursing assistants.
• Complete required charts, treatment plans, and orders.
• Admit, transfer, and discharge residents as needed.
• Make rounds with physicians as requested.
• Orient new residents and families to the facility.

• Participate in developing and adjusting care plans.
• Background check clearance required.

• Prepare and administer medications as ordered by the physician
• Make rounds with physicians as requested/necessary.
• Orient and welcome new residents and families.
• Answer call lights and provide nursing assistance.
• Participate in and make recommendations for care plans.
• You will need to have a current California Nursing License
