Salary Range: $38.28 - $38.28 Hourly

Travel Percentage: Negligible

Job Shift: Any

Shifts available are PT, 8am-8pm, and 8pm-8am.

Role and Responsibilities

• Ensure the well‑being of the individual.
• Provide appropriate care in accordance with personal‑centered plan and all physician orders.
• Provide home care nursing to an adult in home and community settings.
• Create a self‑centered environment for the individual – focused on their hopes, dreams, and goals.
• Follow all care plans, ISP, and protocols.
• Ensure that the individual’s home and medical equipment are clean and safe.
• Lead for ensuring individual’s health maintenance.

Required Skills

• Excellent Documentation skills
• Proficient in G‑Tube and J‑Tube care and medication administration
• Ventilator experience
• Tracheotomy experience
• Use of appropriate policies and systems as required for this position.

Qualifications and Education Requirements

• Current unencumbered nursing license (LPN) in state of PA
• Current CPR
• Minimum 1 year LPN homecare experience
• Minimum 1 year Trach, Vent, and G Tube experience
• Graduate of an accredited nursing program
