Job summary

Rose View Nursing and Rehabilitation Center in Williamsport, PA, is seeking a part-time Licensed Practical Nurse Assessment Coordinator (LPNAC) with a strong MDS background to join our clinical team.

Ideal candidates have at least two years of MDS experience in a skilled nursing facility, a valid Pennsylvania LPN license, and excellent organizational, communication, and collaboration skills to support care planning and regulatory compliance.

Qualifications

• Current, active Pennsylvania LPN license in good standing.
• Minimum of two years of MDS experience in a skilled nursing facility.
• Knowledge of MDS assessment processes, care planning, and regulatory requirements.
• Strong organizational skills and attention to detail.
• Excellent communication and collaboration skills.
• Ability to manage deadlines and prioritize responsibilities effectively.

Responsibilities

• Coordinate and complete Minimum Data Set (MDS) assessments in accordance with applicable state and federal regulations.
• Ensure assessments are completed accurately and within required timeframes.
• Collaborate with nursing staff, the interdisciplinary team, and other departments to support comprehensive resident assessments and care planning.
• Assist with developing, reviewing, and updating individualized resident care plans based on assessment findings.
• Monitor assessment schedules and maintain accurate documentation.
• Support compliance with applicable Centers for Medicare & Medicaid Services (CMS) guidelines and facility policies.
• Identify assessment-related issues and communicate with the appropriate team members to support timely resolution.
• Maintain organized records and documentation to support regulatory compliance and quality resident care.
