Job summary

CRESWELL is seeking an experienced LPN/LVN to lead the nursing services in a long-term care facility. You will direct nursing assistants, ensure compliance with policies, and oversee charting, medication administration, and discharge planning in a collaborative, patient-centered environment.

Responsibilities include supervision of staff, ensuring QA participation, and coordinating with physicians and department teams to provide high-quality resident care.

Qualifications

• Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program.
• Must possess a current, unencumbered, active license to practice as an LPN/LVN in this state.
• Knowledge and experience with PCC preferred.

Responsibilities

• Direct the day to day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the long term care facility.
• Write resident charge slips and forward to the Business Office; maintain the Daily Census Report and submit to the Business Office as required.
• Inform nursing personnel of new admissions, ensure rooms are ready, and greet newly admitted residents; coordinate care plans with physicians as required.
