Job summary

Creswell Post Acute in Creswell, OR is seeking an experienced LPN/LVN shift supervisor to lead nursing services and coordinate patient care across the unit. You will plan staffing, oversee daily rounds, ensure adherence to policies, and participate in quality assurance activities to maintain high standards of resident care.

Responsibilities include charting, medication administration oversight, discharge planning support, staff training, and ensuring regulatory compliance.

Qualifications

• Must possess a nursing degree or be a graduate of an approved LPN/LVN program.
• Must hold a current, active LPN/LVN license in Oregon.
• Strong leadership, supervisory, and communication skills.
• Experience in long-term care nursing and care planning.
• Ability to interpret policies and coordinate resident care with physicians.

Responsibilities

• Direct day-to-day nursing services and ensure compliance with policies.
• Plan shifts and assign nursing assistants and support personnel.
• Oversee charting, medication administration, and accurate documentation.
• Participate in discharge planning and quality assurance activities.
• Provide staff training and performance feedback to improve care.
• Maintain warm, calm environment and effective interdepartmental collaboration.
