The Wellness Collective in Sioux Falls, SD is seeking a Part-Time LPN to support functional medicine and psychiatric providers with personalized, whole-person care. Typical day includes patient prep, care coordination, and between-visit communication, with about 16-24 in-person visits per day.

No evenings or weekends; this role offers a slower pace than hospital settings and an opportunity to build ongoing patient relationships while working with a collaborative clinical team.

Qualifications

• Active South Dakota nursing license.
• Strong written and verbal communication skills.
• Excellent attention to detail and organizational skills.
• Ability to prioritize and manage multiple tasks in a busy outpatient environment.
• Ability to work independently while contributing to a collaborative clinical team.
• Comfort using an EMR

Responsabilités

• Prepare patients for appointments by obtaining and documenting vital signs and reviewing medications, supplements, allergies, and preferred pharmacy information.
• Support 16–24 in-person patient visits per day across functional medicine and psychiatric services.
• Manage and respond to a significant volume of patient correspondence, including phone calls and secure messages.
• Communicate with patients regarding treatment plans and escalate concerns to providers when needed.
• Complete medication prior authorizations and coordinate prescriptions with pharmacies.
• Document patient interactions and care coordination in the EMR.
