Job summary

St. Joseph Residential Care Home is seeking a compassionate LPN for a part-time day/evening shift, including every other weekend. The role focuses on delivering high-quality care to active seniors in a supportive, team-based setting.

Responsibilities include administering medications, assisting with ADLs, following individualized care plans, documenting outcomes, and ensuring a safe, homelike environment that supports resident independence and dignity.

Qualifications

• Current LPN license required.
• Strong attention to detail and careful documentation.
• Excellent communication and organizational skills.
• Ability to collaborate effectively within a multidisciplinary team.

Responsibilities

• Assist residents with ADLs including bathing, dressing, grooming, toileting, and mobility.
• Administer medications accurately and safely per guidelines.
• Follow individualized care plans and document care and outcomes.
• Monitor changes in resident condition and report promptly to supervisors.
• Maintain a safe, welcoming home environment supporting independence and dignity.
• Engage residents to promote meaningful daily experiences.
• Complete charts, reports, logs, and records on time.
