Job summary

St. Joseph Residential Care Home is hiring a full-time evening LPN to deliver compassionate, resident-centered care to active seniors. You will administer medications, assist with daily living tasks, and document care accurately in coordination with the nursing team.

Qualifications

• Active LPN license
• Experience in senior living or long-term care preferred
• Strong attention to detail with good communication
• Ability to collaborate effectively within a multidisciplinary team
• Compassionate, resident-centered approach with confidentiality and safety

Responsibilities

• Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support.
• Administer medications accurately and safely according to guidelines and policies.
• Provide care per individualized care plans and document outcomes.
• Monitor and report changes in resident condition to nursing leadership and families as needed.
• Maintain a safe, welcoming, homelike environment addressing physical, emotional, and psychosocial needs.
• Engage residents to promote independence, dignity, and meaningful daily experiences.
• Complete thorough and timely documentation in charts, reports, logs, and records.
