Qualifications

• Current and valid LPN license in the state of employment.
• Completion of an accredited practical nursing program.
• Basic Life Support (BLS) certification.
• Demonstrated knowledge of nursing principles and practices relevant to skilled nursing care.
• Ability to communicate effectively with patients, families, and healthcare team members.

Responsibilities

• Administer medications and treatments as prescribed by physicians and according to facility protocols.
• Monitor and record patients’ vital signs, symptoms, and overall condition, reporting any changes to the healthcare team promptly.
• Assist patients with activities of daily living, including hygiene, mobility, and nutrition, while promoting independence whenever possible.
• Collaborate with interdisciplinary team members to develop, implement, and update individualized care plans.
• Maintain accurate and timely documentation of nursing care, patient responses, and any incidents in compliance with regulatory standards.
• Provide emotional support and education to patients and their families regarding care plans and health management.
• Ensure compliance with infection control policies and safety procedures to maintain a safe care environment.
