Qualifications

• Current licensure as a Licensed Practical Nurse (LPN) in the state of Pennsylvania.

Responsibilities

• Provide direct patient care to residents, including administering medications, performing treatments, and assisting with activities of daily living.
• Assess resident needs and develop individualized care plans to meet those needs.
• Monitor resident conditions and report any changes or concerns to the Charge Nurse or other healthcare professionals.
• Maintain accurate and detailed records of resident care, including medication administration and treatment plans.
• Collaborate with other healthcare professionals to ensure comprehensive and effective care.
• Provide emotional support and comfort to residents and their families.
• Maintain a clean and organized work environment, adhering to infection control and safety protocols.
