Job summary

LakeHouse Senior Living in Indiana Lafayette is seeking an LPN to provide nursing care per resident plans, conduct health evaluations, monitor conditions, and coordinate services to promote safety and well-being. Requirements include two years of LPN experience in assisted living or a nursing home and an active LPN license in good standing.

As an LPN, you will provide nursing care in accordance with the client's plan of care to include comprehensive health and psychosocial evaluation, monitoring of the client’s condition, health promotion and prevention coordination of services.
