Woodleigh Chase by Erickson Senior Living in Fairfax, Virginia is currently seeking a part-time licensed clinical social worker to join our physician-led geriatrics practice. You will provide psychosocial assessments, patient and family counseling, and care coordination within a multidisciplinary team.

The successful candidate will have a Social Work degree (BSW or MSW) and a state-licensed LCSW, with a minimum of three years of experience, preferably in health care and geriatric settings.

• Social Work degree required (BSW or MSW).
• Licensed Social Worker with state licensure required.
• Minimum of 3 years social work experience, healthcare setting preferred.
• Knowledge of geriatric population, case management and dementia preferred.

Compensation

$45,000 - $51,000 per year commensurate with experience (includes base + bonus + incentive)

What you will need
