Job summary

Community Health Systems, Inc. is seeking a Licensed Clinical Social Worker for the Work Well Clinic in Hattiesburg, MS. This part-time, day-shift position offers two days per week with no weekends, focused on delivering psychosocial assessments, counseling, and care planning.

The role collaborates with multiple disciplines to ensure patient and family access to resources and supports effective discharge planning and continued care.

Qualifications

• Master's Degree in Social Work or Psychology required
• 2-4 years of experience in individual, group, and family therapies required
• LCSW- License Clinical Social Worker required
• BCLS - Basic Life Support obtained within 7 days of employment required

Responsibilities

• Conduct psychosocial assessments for adolescent, adult, geriatric, neonatal, infant and pediatric patients.
• Determine inter-relatedness of psychosocial factors and health status/disease process.
• Identify emotional, social and financial needs of patient/family and provide referrals as appropriate.
• Identify treatment plans and follow up with discharge planning team.
• Assist in social work coverage and support patient/family coping with illness or disability.
