Qualifications

• Active Wisconsin RN license
• CPR/BLS certification required
• Strong organizational and communication skills
• Experience with discharge planning or care coordination preferred
• Knowledge of electronic medical records and processes

Responsibilities

• Manage the referral list and onboarding with care team.
• Assist infusion nurses with patient intake.
• Coordinate patient progress toward admission and follow‑up with providers and staff.
• Document patient information in electronic medical records.
• Communicate with referral sources about insurance and financial responsibilities.
• Serve as a resource to home health agencies and discharge planning partners.
• Support onboarding and provide preceptorship for new staff.
• Uphold infection control and safety standards while improving daily processes.
