Job summary

Signify Health in-home physician role offers flexible, part-time work delivering health evaluations at patients' homes across licensed states. You’ll engage patients one-on-one, review medical histories, and perform physical assessments with careful attention to safety and quality.

As a part-time clinician, you’ll travel locally, conduct virtual visits when needed, and provide diagnostic and preventive services as indicated, supporting the health plan's care coordination and patient outcomes.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses preferred.

Responsibilities

• Conduct in-home health evaluations for members.
• Perform medication and medical history review and physical evaluation.
• Travel locally within licensed states as needed; conduct virtual visits when required.
• Provide diagnostic and preventive services as indicated.
• Collaborate with health plans and clinicians to coordinate care.
