Job summary
Lorian Health is seeking a skilled and compassionate LPN/LVN to join their Sacramento Hospice team in Elk Grove. This position requires providing in-home nursing care under RN supervision while supporting patients with daily care needs. Qualifications
- Current LPN/LVN license in the state of practice.
- Graduate of an accredited practical/vocational nursing program.
- Minimum of one year of recent experience in a Home Health or Hospice Setting.
Responsibilities
- Provide nursing care according to RN-developed Plan of Care.
- Monitor and document changes in patient condition.
- Assist with activities of daily living.
- Educate patients and caregivers within LPN/LVN scope of practice.
