Job summary

Suncrest Hospice in Elk Grove Village is seeking a Licensed Practical Nurse (LPN) to provide patient care across home and facility settings as part of an interdisciplinary team.

You will perform psychosocial assessments, coordinate visits, and support patients and families while adhering to state regulations. A valid LPN license in good standing and reliable transportation are required.

Qualifications

• Current Licensed Practical Nurse (LPN) licensure (in good standing) in the state(s) of practice.
• Ability to work well within a team environment to collaborate daily with a team of clinicians.
• Time management skills to manage visits, documentation, meetings and urgent needs.

Responsibilities

• Provide care and services for a group of patients, visiting patients where they reside, including private homes, Skilled Nursing Facilities, and Assisted Living Facilities.
• Contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources.
