Qualifications

• Minimum 6 months of caregiving experience or relevant education/certification (CNA, HHA, MA, or equivalent)
• Access to a smartphone capable of running the CareLinx app
• Ability to communicate in English (bilingual encouraged)
• Reliable transportation to and from client homes
• Must pass a background check
• Program-specific training prior to accepting shifts
• Medical clearance such as TB test, physical, or drug screening
• Valid driver’s license and MVR check for driving shifts

Responsibilities

• Assist clients with activities of daily living (ADLs) including companionship, meals, mobility, bathing/grooming/dressing, medication reminders (non-clinical), and light housekeeping
• Complete and document care tasks in the CareLinx mobile app during each shift
• Monitor and report changes in a client’s condition to the CareLinx clinical team
• Submit timesheets on time through the CareLinx app
• Communicate with the CarePartner about case updates or client changes
• Ensure patient privacy, dignity, and well-being in a safe environment
• Maintain client confidentiality at all times
• Additional shift opportunities based on client needs: transportation and errands, hospital discharge escort, telehealth navigation
