Job summary

Valor Hospice in Dallas, TX is seeking a part-time Social Worker (MSW) to lead psychosocial assessments and coordinate patient and family support. The role collaborates with the Interdisciplinary Group/Team and participates in on-call rotations as needed to ensure comprehensive palliative care.

Responsibilities include facilitating advance directives discussions, family counseling, and discharge planning while maintaining privacy and compassionate care aligned with hospice philosophy.

Qualifications

• MSW degree or approved social work degree with supervised path.
• 1 year of social work experience in healthcare.
• Current state social work licensure or licensure requirement.
• Valid driver’s license.
• Ability to coordinate care with IDG/Team and families.
• Excellent interpersonal and written communication skills.

Responsibilities

• Complete Comprehensive Assessment within five days of patient election.
• Coordinate care with IDG/Team and on-call rotation.
• Counsel patients and families on end-of-life and grief.
• Attend IDG/Team meetings and participate in care planning.
• Promote hospice philosophy and quality of care.
