Job summary

Sanctuary Hospice is seeking a dedicated Licensed Master Social Worker (LMSW) or MSW to provide compassionate social work services to hospice patients and families in Summit, Portage, Stark, Wayne, and Medina counties.

This part-time role runs Monday–Friday, 8:30 am–2:30 pm, focusing on psychosocial assessment, counseling, care planning, and collaboration with the RN Case Manager and Bereavement Coordinator. A valid state license and reliable vehicle are required.

Qualifications

• Must have MSW from an accredited program.
• Active social work license in the state.
• Driver's license, vehicle insurance, and access to a dependable vehicle.

Responsibilities

• Assess psychosocial status and communicate with the RN Case Manager and interdisciplinary team.
• Evaluate medical needs, home situations, financial resources, and community supports.
• Develop and implement individualized social work plans with the Bereavement Coordinator.
• Participate in interdisciplinary meetings and support the care team.
• Provide empathetic counseling to patients and families.
