Job summary

Sanctuary Hospice is seeking a dedicated Licensed Medical Social Worker (LMSW/ MSW) for a part-time role in the Summit, Portage, Stark, Wayne, and Medina County area. You will deliver compassionate social work services to hospice patients and families, provide counseling, and support emotional well-being in challenging times.

The role requires an MSW, state licensure as a Social Worker, a valid driver’s license, and at least one year of healthcare or hospice social work experience.

Qualifications

• Requires Master of Social Work from an accredited school of social work.
• Licensed Social Worker in good standing with the State Board of Social Work.
• Minimum one year of healthcare/hospice social work experience preferred.

Responsibilities

• Assess psychosocial status and communicate with the RN Case Manager and interdisciplinary team.
• Evaluate home situations, resources, and community supports to inform care.
• Develop and implement individualized social work plans in coordination with the Bereavement Coordinator.
• Participate in care planning through interdisciplinary meetings and address psychosocial stresses.
• Provide empathetic counseling to patients and families.
