Job summary

Sanctuary Hospice is seeking a dedicated Licensed Master Social Worker (LMSW) to deliver compassionate social work services to our hospice patients and families. This part-time role focuses on counseling, emotional support, and advocacy within a patient-centered team. Schedule: Part-Time, Monday - Friday, 8:30 am - 2:30 pm.

Coverage Area: Summit, Portage, Stark, Wayne, and Medina County. You will collaborate with the RN Case Manager and the interdisciplinary team to assess psychosocial needs,

Qualifications

• Master of Social Work from an accredited program.
• Licensed as a Social Worker by the State Board.
• Current Driver's License and vehicle insurance.
• Minimum 1 year in hospice social work or healthcare setting.
• State-specific license requirements in OH and other listed states.

Responsibilities

• Assess psychosocial status and communicate with the RN Case Manager.
• Evaluate home situations, resources, and community supports.
• Develop and implement individualized social work plans.
• Attend interdisciplinary meetings and support the care team.
• Provide empathetic counseling to patients and families.
