Job summary

Tapestry Hospice & Palliative Care in Georgia is seeking a Part-time Hospice Social Worker to provide emotional and practical support to patients and families facing serious illness. You will collaborate with the Interdisciplinary Team and focus on psycho-social aspects of care throughout the hospice experience.

The role requires GA Social Work licensure, strong communication skills, and a compassionate approach to end-of-life care.

Qualifications

• Licensed as a Social Worker through the Georgia Board
• Strong verbal and written communication skills
• History of building patient and family relationships
• Holds a valid driver’s license, car insurance, CPR certification, and positive driving record
• Hospice, home health, or hospice experience is extremely preferred
• Part-time availability

Responsibilities

• Assess patient/family social and emotional factors to estimate their potential to cope with terminal illness and death.
• Provide psycho social counseling and support to the patient/family experiencing emotional, social, and economic conflict.
• Participate in Interdisciplinary Team (IDT) meetings to establish, review, and revise the plan of care for each patient, including exchange of information, review of problems, and sharing of professional support.
• Maintain the dignity of the dying patient.
