Qualifications

• Licensed as a Social Worker through the Georgia Board.
• Possess effective communication skills (including strong verbal and written).
• Possesses a history of building and influencing patient and family relationships.
• Holds a current driver's license, car insurance, CPR certification, and positive driving record.
• Hospital, home health, or hospice experience is extremely preferred.
• Part-time availability.

Responsibilities

• Assess patient/family psychosocial factors to estimate coping with terminal illness.
• Provide psychosocial counseling and support to patients and families.
• Participate in Interdisciplinary Team (IDT) meetings to establish, review, and revise the plan of care.
• Maintain the dignity of the dying patient.
