Job summary

AdvisaCare Hospice invites experienced LPNs/RNs to provide one-on-one home hospice care with empathy and dignity. The role emphasizes compassionate presence in patients' homes, coordination with families, and participation in interdisciplinary teamwork to ensure comfort and quality of life.

The position offers flexible hours (8–16 hours per week) and an on-call rotation, with supervision visits, team meetings, and ongoing training to support meaningful end-of-life care.

Qualifications

• Current Michigan License in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Willingness to participate in field visits daily and attend office meetings.
• Ability to adapt to patient status and full-time on-call requirements.

Responsibilities

• Initial and ongoing assessment of patient needs (physical, functional, psychosocial, environmental).
• Implement the individualized plan of care and revisions as needed.
• Ensure compliance with AdvisaCare Hospice Policies and Medicare rules.
• Educate patient and family about disease, self-care, end-of-life care, and ethical issues.
• Supervise paraprofessionals and participate in IDG conferences.
• Contribute to performance improvement programs and maintain high client satisfaction.

FLEXIBLE HOSPICE OPPORTUNITY

We’re looking for a flexible RN or LPN to provide hospice care 8–16 hours per week, including a rotation of the on-call schedule.

As a member of the AdvisaCare Hospice team, you’ll provide emotional support with the goal of helping patients experience their final days with dignity and comfort.

• Appropriate supervisory visits and clinical observation

• Maintain high level of customer satisfaction with our clients, families and caregivers
• Communication of information using current process and technology within organization

Requirements:

• Current Michigan License in good standing
• Minimum 1 year hospice experience

• Ability to adapt to patient’s status and needs on a full time on call basis
