Job summary

AdvisaCare Hospice in Michigan is seeking a Full-Time Hospice RN to provide compassionate, in-home care and support for patients and families.

This role emphasizes presence, dignity, comfort, and personalized care, with 30–40 hours per week and on-call rotation to fit family schedules while sustaining meaningful impact. You will collaborate with an interdisciplinary team, participate in care planning, supervise paraprofessionals, and advocate for patient and family wishes.

Qualifications

• Current Michigan license in good standing required.
• Minimum 1 year hospice experience preferred.
• Empathetic approach to patients and caregivers.
• Willingness to participate in daily field visits and on-call schedule.
• Ability to adapt to patient status and meet needs on a full-time on-call basis.

Responsibilities

• Provide one-on-one care in patients' homes with presence and compassion.
• Support patients and families with dignity and comfort.
• Conduct supervisory visits and education to assigned team members.
• Participate in interdisciplinary group meetings and care planning.
• Maintain accurate clinical and progress notes and IDG conference participation.
• Advocate for patient and family needs and wishes.

As a Hospice RN with AdvisaCare Hospice, you’ll provide emotional support and compassionate clinical care, helping patients experience their final days with dignity, comfort, and respect.

You’ll also:
Conduct appropriate supervisory visits
Provide clinical observation and feedback to assigned team members
Participate in interdisciplinary group meetings
Help develop, review, and revise patient/family plans of care
Advocate for the needs and wishes of patients and their families

• Initial and on-going assessment of patient’s physical, functional, psychosocial and environmental needs
• Implement the individualized plan of care and revisions as needed
• Compliance with AdvisaCare Hospice Policies and Procedures, Medicare compliance
• Consulting with and educating the patient/family on disease process, self-care techniques, end-of-life care, dealing with issues of ethical concern
• Supervision of paraprofessionals providing services to patient
• Preparing clinical and progress notes, participating in IDG conferences
• Participate in Hospice performance improvement program
• Maintain high level of customer satisfaction with our clients, families and caregivers
• Communication of information using current process and technology within organization

Requirements:

• Current Michigan License in good standing
• Minimum 1 year hospice experience
• Empathetic approach to patients and caregivers
• Participate in field visits daily, weekly office meetings as indicated
• Ability to adapt to patient’s status and needs on a full time on call basis
