Job summary

AdvisaCare Hospice is hiring a RN or LPN for Hospice at Home in Michigan. You will provide compassionate, one-on-one care at patients' homes, supporting families with dignity and comfort. The role includes supervision of aides, interdisciplinary feedback, and careful planning of the patient/family care plan.

The position spans full-time or part-time options, with emphasis on on-going assessment, education, and delivering high-quality hospice care in the home setting.

Qualifications

• Current Michigan license in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Participate in field visits daily and at meetings as indicated.
• Ability to adapt to patient status on a full-time on-call basis.

Responsibilities

• Initial and on-going assessment of patient’s physical, functional, psychosocial and environmental needs.
• Implement the individualized plan of care and revisions as needed.
• Compliance with AdvisaCare Hospice policies and procedures, Medicare compliance.
• Educate patient and family on disease, self-care, end-of-life care, and ethics.
• Supervision of paraprofessionals providing services to patient.
• Prepare clinical and progress notes, participate in IDG conferences.
• Participate in Hospice performance improvement program.
• Maintain high level of customer satisfaction with clients, families and caregivers.
• Communicate information using current processes and technology within the organization.

Are you looking for a challenging and rewarding position as a Full time/Part time Hospice RN or LPN ? We are searching for compassionate, dynamic, energetic, organized and efficient professionals with Hospice experience to become part of our Hospice Family for our Lakeview/Edmore/Mecosta areas

For this role, you need to be a registered nurse who sees the value of Hospice care. You will be providing emotional support, with the goal to help patients live their final days with dignity and comfort. Our Hospice RN's will conduct appropriate supervisory visits which allow for clinical observation of performance of assigned team members. Participate and provide feedback to the team for regular interdisciplinary group meetings; on the development, review and revision of the patient/family plan of care.
