Job summary

OnePoint Patient Care, a PharMerica affiliate, is hiring a part-time Pharmacist in Vancouver, WA to support hospice patients in a non-retail setting. You’ll work with a dedicated team, review prescriptions, and collaborate with clinicians to optimize therapy.

Qualifications

Bachelor’s Degree in Pharmacy; PharmD preferred and current state license required.
Knowledge of drug therapy guidelines, medication safety, and regulatory standards.
Proficiency with electronic medical records and pharmacy information systems.
Strong clinical judgment and effective communication skills.

Responsibilities

Receive and evaluate prescriptions for drug appropriateness, correct dosage, interactions and administration.
Collaborate with physicians, nurses and other healthcare professionals to support care.
Maintain up-to-date patient profiles including medical history and medications.
Provide clinical recommendations on medication therapy changes and optimizations.
Ensure safe procurement, storage, and disposal of medications per policy and regs.
Comply with state and federal regulatory requirements related to pharmacy practice.
Supervise automated machines and IV preparation where applicable; support dispensing systems.
