Job summary

Qualifications

• Meets the training requirements of the State.
• Completed a minimum of 75 hours of classroom and supervised practical training.
• At least 18 years of age.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and comfort providing care to the terminally ill.
• Satisfactory references from previous employers.
• Self-directed with minimal supervision and calm demeanor in patient's home.
• Shows empathy for the needs of the ill.
• Demonstrates tact and good hygiene.
• Licensed driver with insured vehicle in good condition.

Responsibilities

• Perform personal care tasks and attend to patient requests promptly.
• Provide patient and family with compassionate communication and confidentiality.
• Ensure safety and proper use of equipment (e.g., O2, rails).
• Assist with bathing, grooming, feeding and daily activities as assigned.
• Support mobility, transfers, and range of motion per supervision.
• Assist with medications as allowed by state regulations (reminders).
• Maintain clean and orderly living area and complete documentation.
• Participate in quality improvement activities as needed.
