Job summary

4Ever Young in Gilbert, AZ is seeking an experienced Nurse Practitioner or Physician Assistant to lead hormone and metabolic health programs. The role includes bio-identical hormone therapy, peptide therapy, medical weight loss, and IV vitamin therapy.

The candidate will guide patients from consultation to personalized treatment plans, with emphasis on patient education and compliance. The position is 24 to 32 hours per week, on-site, with no late nights or weekends required.

Qualifications

• Active, unrestricted Arizona NP or PA license.
• Active DEA registration with prescribing authority.

Responsibilities

• Conduct in-depth consultations with new patients and review health histories.
• Perform physical exams and order blood draws.
• Develop personalized treatment plans based on symptoms and labs.
• Prescribe and manage medications, including controlled substances, per DEA and state rules.
• Perform pellet insertions for hormone therapy patients.
• Maintain HIPAA-compliant charting and documentation.
