Job summary

Optum is seeking a part-time Home Infusion Nurse in California to provide clinical care and education to home-based patients receiving infusion therapy. The role involves independent work in patient homes, AIS settings, and other care environments, with focus on safety, documentation, and collaboration with a multi-disciplinary team.

Requirements include active RN licensure in California and 1+ year of home health infusion experience.

Qualifications

• Active and unrestricted RN licensure in California.
• 1+ year RN home health infusion experience or similar setting.
• Proven ability to work with an interdisciplinary team and independently in home or alternate-site settings.

Responsibilities

• Establish clinical rapport with patients and families while delivering infusion therapy.
• Assess patient suitability for home infusion and safety of home environment.
• Document patient history, plans of care and required forms timely.
• Perform nursing tasks per physician orders and company protocols, monitor therapy and patient status.
• Coordinate with care team, educate patients and caregivers, and ensure compliance with HIPAA and OSHA.
