Job summary

Qualifications

• Active and unrestricted RN licensure in California.
• 1+ year RN experience in home health or infusion settings.
• Proficient in electronic medical record documentation.
• Ability to work with an interdisciplinary team of professionals.
• Willing to maintain a flexible part-time schedule (20 hrs/week).

Responsibilities

• Deliver infusion therapy and patient education in home or ambulatory settings.
• Assess patient suitability for home infusion and home environment safety.
• Obtain medical history and update treatment plans as needed.
• Complete nursing plans, POT/485s, orders and visit notes with timely documentation.
• Collaborate with care team to plan and coordinate patient care.
• Provide skilled nursing care per physician orders and plan of treatment.
• Manage vascular access devices and perform dressing changes as prescribed.
• Administer medications and monitor vital signs and adverse reactions.
• Communicate patient status to the care team and supervisors.
