Iowa Home Care is seeking a compassionate Registered Nurse (RN) to join our team as a Part-Time Home Health Wound Care Nurse in West Des Moines, IA. You will manage wound assessments, treatments, and ongoing clinical oversight in patients’ homes, collaborating with physicians and families to optimize healing.

The ideal candidate has an Iowa RN license, at least one year of nursing experience (home health or wound care preferred), strong clinical documentation skills, and CPR certification.

Qualifications

• RN license in the State of Iowa.
• Minimum of 1 year of nursing experience required.
• Home health and/or wound care experience preferred.
• Strong assessment, critical thinking, and clinical documentation skills.

Responsibilities

• Perform comprehensive nursing assessments and develop individualized plans of care.
• Assess, monitor, and treat wounds including surgical, pressure, diabetic, vascular, and traumatic wounds.
• Perform wound measurements, staging, documentation, and evaluation of healing progress.
• Administer wound care treatments per physician orders and evidence-based practices.
• Educate patients, families, and caregivers on wound care, prevention, nutrition, and disease management.
• Monitor for infection or complications and communicate changes to the physician and care team.
• Coordinate care with physicians, therapists, aides, and other healthcare professionals.
• Complete Start of Care, Recertification, Resumption of Care, and Discharge assessments as assigned.
• Ensure accurate, timely, compliant documentation within the EMR.
• Participate in case conferences and interdisciplinary team meetings.
• Maintain compliance with federal, state, and agency regulations.
