Trinity Health Mid-Atlantic seeks a Contingent Speech-Language Pathologist for home health. Provides diagnostic and therapeutic services, with supervision of HHAs and precepting new staff. Flexible schedule, 24 hours per week on average, 3 days weekly, part-time.

Contingent Speech-Language Pathologist (Home Health) Provides professional speech and audiology diagnostic and therapeutic services to clients as authorized by physicians and in compliance with state practice acts and licensure requirements. Supervision Serves as preceptor for new staff and provides functional supervision of HHAs.
Prioritize and plan visits based on client condition, physician orders, and available resources.
Maintain knowledge of laws, regulations, ethics, and agency policies.
Assess clients at admission and required intervals using standardized documentation.
Develop and revise treatment plans with physician and client, including attainable goals.
Make referrals and coordinate care with other disciplines.
Apply professional judgment in evaluations and interventions.
Implement preventive, therapeutic, and rehabilitative procedures per treatment plan.
Educate clients and families on communication and swallowing improvement.
Report changes in client condition promptly and obtain updated orders.
Participate in quality improvement and peer review.
Document all care accurately and timely in the point-of-care system.
Ensure safe home environment and discharge planning.
Minimum Qualifications Meets the educational and experience requirements for a Certificate of Clinical Competence in Speech Pathology granted by the American Speech and Hearing Association; or, meets the educational requirements for certification and is in the process of accumulating the supervised experience required for certificate.
Occasional lifting, standing, walking, bending, and positioning patients. Ability to manage detailed tasks with frequent interruptions. Adaptable to changing work parameters; travel required.
