Job summary

• Bachelor's degree in Speech Language Pathology required.
• Master's degree in Speech Language Pathology preferred.

Responsibilities

• Develop speech therapy plans of care after physician referral.
• Visit patients for assessment and evaluation; complete OASIS assessments.
• Provide education to team members, patients, and families.
• Document care and maintain medical records in a timely manner.
• Participate in agency quality improvement programs.
• Collaborate with care providers and discharge planning as needed.
