Job summary

Ascension at Home, in collaboration with Compassus, seeks a Home Health Speech Language Pathologist for PRN coverage. The role focuses on evaluating and treating speech, language, cognitive, and swallowing disorders in home health settings under physician direction.

The ideal candidate has a ASLP license in the state and a minimum of one year of home health experience, with strong communication and patient education skills. Flexibility and compliance with regulatory standards are essential.

Qualifications

• Bachelor's degree in Speech Language Pathology required.
• Master's degree in Speech Language Pathology preferred.
• Minimum of one year of experience in a home health setting required.

Responsibilities

• Provide care utilizing infection control measures (OSHA).
• Accept clinical assignments consistent with education and competence.
• Monitor cases for third-party payor requirements.
• Communicate with patient/family and care team to coordinate services.
• Document plan of care and outcomes and ensure quality of care.
• Supervise Home Health Aide/UP to ensure service quality.
• Provide services requiring specialized speech therapy skills.
• Educate client/family about disease process.
• Perform initial and ongoing speech, language, cognitive, and swallowing assessments and set goals.
• Develop and revise the plan of care in consultation with the physician and care team.
