Qualifications

• Current and unrestricted RN license in state of practice.
• Current driver's license and reliable transportation.
• CPR certification preferred or ability to obtain within 90 days.
• Ability to work independently and manage time effectively.
• Strong written and verbal communication skills.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and LPNs/LVNs, provides instruction, and assigns tasks per state/federal regulations.
• Provides required supervisory visits and ensures plan of care compliance.
• Completes comprehensive assessments (OASIS) and accurate medication reconciliation.
• Documents patient visits per policy and payer requirements and synchronizes with policy.
