Job summary

Advantage Home Health Services in Bethlehem, PA is seeking a part-time Home Health Registered Nurse to assess patients in their homes and develop care plans tailored to individual needs. You’ll conduct Start of Care visits, complete OASIS timely, reconcile medications, and ensure accurate orders for home health services are in place.

You will document clearly, coordinate with the Case Manager, and educate patients and families on care plans and treatments.

Qualifications

• An active RN license in the state in which practicing or New Grad with eligibility to take NCLEX.

Responsibilities

• Conduct Start of Care comprehensive assessments and complete OASIS timely.
• Draft care plans, reconcile patient medications, and ensure orders for care are accurate and complete.
• Prepare accurate, orderly, and timely documentation.
• Coordinate care with patient and provide proper reports to Case Manager.
• Provide health care instructions to the patient/family as appropriate per assessment and plan.
