Job summary

Elite Care Management, Inc. in Sleepy Hollow is seeking a Part-Time Registered Nurse or Licensed Practical Nurse to provide one-on-one home health care. You will care for one patient per shift, observe health conditions, administer medications, and coordinate with the care team to support daily living activities.

Shifts are Saturdays and Sundays, 10am–10pm, offering competitive pay and full benefits. This role emphasizes compassionate, personalized patient care in the home setting.

Qualifications

• Must have driver’s license.
• Must have Social Security Card.
• Must have Auto Insurance Card.
• CPR certification required.
• RN or LPN license required.
• TB test results within specified window.

Responsibilities

• Observe patient, document conditions and reactions, and notify supervisor or physician.
• Administer medications and treatments as prescribed and monitor effectiveness.
• Demonstrate nursing policies and procedures.
• Support patient care plan and update as needed.
