Job summary

Elite Care Management is seeking a part-time Registered Nurse or Licensed Practical Nurse to provide one-on-one home health care. You will work with a single patient per shift, supporting daily living activities and comfort in the patient’s home, and you’ll implement the care plan with attention to the patient’s needs.

Responsibilities include monitoring health conditions, administering medications, and coordinating with supervisors.

Qualifications

• Must hold RN or LPN license in good standing.
• Experience in home health care is preferred.
• Excellent communication skills required.

Responsibilities

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient’s condition and reaction to drugs, treatments, and significant incidents.
• Demonstrate understanding organizational and nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
