Job summary

BrightStar Care is seeking a part-time Physical Therapist to join our Therapies Team in Duluth, MN. You will assess, plan, implement, and evaluate PT modalities in home health with a patient-centered approach.

Applicants must be a graduate PT with MN licensure, CPR certified, and have at least four years of PT experience. A valid driver’s license and computer skills are required. BrightStar Care offers a supportive environment and flexible scheduling.

Qualifications

• Graduate of PT program recognized by APTA or equivalent.
• Active license to practice Physical Therapy in MN.
• Minimum of four (4) years’ experience as a Physical Therapist; home care a plus.
• Criminal background check required.
• CPR certification current.
• Active driver’s license with clean driving record.
• Computer skills.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Assess, plan, implement, and evaluate PT modalities for patients.
• Adhere to department policies and competency guidelines.
• Improve or minimize residual disabilities.
• Return patient to optimum productive level within capabilities.
• Collaborate with home care staff on care planning.
• Take initial history and perform initial evaluation.
• Consult with physicians on patient progress.
• Participate in QA and performance improvement activities.
• Prepare clinical and progress notes; revise plan of care as needed.
• Consult with family and agency personnel.
• Regularly perform bending, lifting, and standing.

Key Responsibilities

• Assessment, planning, implementation, and evaluation of all physical therapy modalities to patients.
• All duties and standards will be performed according to established policies, procedures, and competency guidelines within the department and organization.
• Improves or minimizes residual physical disabilities of the patient.
• Returns the individual to optimum and productive level within patient’s capabilities.
• Participates with all other home care personnel in patient care planning.
• Takes initial history and makes initial evaluation.

• Ability to do extensive bending, lifting and standing on a regular basis.

Requirements

• A graduate of a PT Program recognized by the American Physical Therapy Association or other National accepted Council or Association
• An active license to practice Physical Therapy in MN
• A minimum of four (4) years’ experience as a Physical Therapist, home care experience a plus
• Must have criminal background check
• Must have current CPR certification
• Must have active driver's license and clear driving record
• Computer skills.
• Ability to exercise initiative and independent judgment
