BAYADA Home Health Care is seeking a Home Health Occupational Therapist to provide skilled OT services in Carlisle, Pennsylvania, on a part-time, per-visit basis. The role focuses on comprehensive in-home evaluations, treatment planning, and patient education within a multidisciplinary team.

Responsibilities include visiting clients, assessing function, and coordinating with nurses and therapists to support safe, independent living. Pennsylvania licensure is required.

Formación

• A current Pennsylvania Occupational Therapist license.
• Minimum one year of recent OT experience.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.

Responsabilidades

• Make home visits to clients in designated Cumberland County territories.
• Evaluate patients’ functional status and ADL/IADL performance.
• Develop and revise individualized intervention plans.
• Assess home environment for hazards and necessary adaptations.
• Identify equipment needs and environmental modifications.
• Educate clients and families on rehabilitative care and safety.
• Document observations and interventions in an EMR system.
