Boost Home Healthcare - Greater Boston is seeking a compassionate Registered Nurse to join as a part-time Home Health Case Manager in Massachusetts. You will manage a caseload, perform OASIS assessments, and coordinate with physicians, therapists, and social workers to ensure timely, high-quality home care.

Qualifications

Responsibilities

Manage the Care Journey: primary contact for a caseload of home health patients and coordinate services.
Conduct OASIS assessments and develop individualized care plans with the interdisciplinary team.
Coordinate services with physicians, nurses, therapists, social workers, and home health aides.
Monitor patient progress, update care plans, and communicate with families and caregivers.
Maintain accurate and timely clinical documentation per Medicare guidelines.
Advocate for patients by providing education and resources to promote independence.

What You'll Be Doing

Manage the Care Journey: Serve as the primary point of contact for a caseload of home health patients, ensuring seamless coordination of services.
Conduct Comprehensive Assessments: Complete OASIS assessments and develop individualized care plans in collaboration with the interdisciplinary team.
Coordinate Services: Work with physicians, nurses, therapists, social workers, and home health aides to ensure timely delivery of care and optimal outcomes.
Monitor and Evaluate: Track patient progress, update care plans, and communicate with families and caregivers as needed.
Documentation: Maintain accurate and timely clinical documentation in accordance with Medicare guidelines and agency policy.
Advocate for the Patient: Provide education, resources, and emotional support to patients and their families to promote independence and wellness.

What We’re Looking For

License: RN (Registered Nurse) with active MA state license.
Experience: At least 1 year of experience in home health or case management preferred. Expertise in administering IVs and medications.
Skills: Strong critical thinking, communication, time management, and the ability to work independently.
Tech-Savvy: Comfortable using EHR systems (experience with Kantime, Axxess, HCHB, or WellSky is a plus).
Reliable Transportation: Valid driver’s license, auto insurance, and access to a dependable vehicle.
