Residential Home Health and Residential Hospice is seeking a part-time Medical Social Worker (MSW) to provide skilled social work services in patients' homes in the Parkesburg, PA area. You will join an interdisciplinary team, support families, and develop care plans addressing emotional and environmental factors.

Requires a Master’s in Social Work, 1–2 years in healthcare social work, and a valid driver’s license with about 20% travel. Equal opportunity employer with strong growth opportunities.

Formación

• Master’s in Social Work from graduate school accredited by the Council of Social Work
• One to two years post graduate social work experience in a health care setting
• Licensed MSW preferred
• Valid driver’s license and ability to spend approximately 20% of workday driving to patient locations

Responsabilidades

• Deliver skilled, patient-centered care in a home health setting, collaborating with an interdisciplinary team
• Conduct comprehensive psychosocial assessments and develop individualized care plans
• Educate and support patients and families by connecting them with community resources and care planning
• Maintain accurate, timely EMR documentation while working independently in the field
• Travel to patient homes within assigned territory
