Job summary

Northshore Home Health Care is seeking a dedicated registered nurse to provide skilled nursing care to patients in their homes under the supervision of a RN. You will collaborate with nursing and medical staff to support individualized care plans and serve as a key point of contact for patient needs.

Responsibilities include assessing patient conditions, administering procedures, coordinating with physicians, educating patients and caregivers, and documenting observations and outcomes.

Qualifications

• Nursing licensure/credential for home health nursing required.
• Strong clinical assessment and monitoring abilities.
• Ability to follow care plans and operate under supervision guidelines.
• Clear communication and compassionate, patient-centered approach.
• Reliable documentation skills with attention to detail.

Responsibilities

• Assess and monitor patient conditions; report changes to the RN.
• Administer specialized nursing procedures per treatment plans.
• Coordinate with RNs and physicians to ensure continuity of care.
• Educate patients and caregivers on care instructions, health maintenance, safety.
• Document observations, interventions, and outcomes accurately and promptly.
