Job summary

United Methodist Communities HomeWorks division in Northern New Jersey is seeking a part-time Home Care Registered Nurse (RN) to join our team. The role offers $44.00 per hour with a 9am–2:30pm schedule, four days per week, with flexibility.

The RN will perform in-home visits, coordinate with Home Health Aides and the care team, develop personalized service plans, and provide ongoing nursing support to clients in their homes while maintaining detailed documentation and strong client

Qualifications

Active, unencumbered New Jersey RN license (or multistate license).
1-2 years of RN experience in home care, home health, hospice, or community nursing.

Responsibilities

Complete in-home visits to review client needs and promote well-being.
Collaborate with Home Health Aides and care team to support a positive client experience.
Create and update personalized service plans based on client goals and changing needs.
Provide ongoing nursing visits and follow-up evaluations to support continuity of care.
Build strong relationships with clients and families while serving as a main point of contact.
Maintain timely documentation and communicate updates with the clinical team.
