Job summary

Qualifications

• Active, unencumbered New Jersey RN license or multistate compact RN license.
• At least 2 years RN experience in home care, home health, hospice, or community nursing.
• Valid driver's license with reliable transportation.

Responsibilities

• Visit clients at home to assess their needs.
• Write and update personal care plans.
• Supervise and support CHHAs.
• Help match clients with the right caregiver.
• Document each visit clearly and on time.
• Keep families and the care team up to date.
