Responsibilities

• Maintains the roles and responsibilities of a Genesis/HIV care manager.
• Provides support to the other care managers.
• Oversees QI and CQI for their site and works with Program Coordinator and Director on grant requirements.
• Attends case conference meetings to ensure continuity of services for HIV+ patients.
• Assists in supervision of program peers and Genesis Care Managers when applicable.
• Orients social work interns, peers and care managers to the site.
• Assists with development of community-based resources and collateral agency relationships.
