Job summary

Wilder in Saint Paul, MN is seeking a Health Care Tech Customized Living Aide to provide person-centered daily living assistance to older adults and adults with chronic conditions. Qualifications

• High School Diploma or equivalent required.
• Certified Nursing Assistant, Home Health Aide Certificate or Trained Medication Assistant required.
• Prior experience working in a health care setting with older adults and/or disabled adults preferred.

Responsibilities

• Provide daily living assistance, including housekeeping, laundry, and personal care.
• Administer and manage medications as directed.
• Promote client safety and implement care plans.
• Document care and communicate with an interdisciplinary team.
