Job summary

Amherst H. Wilder Foundation in St. Paul, MN is seeking a Health Care Tech Customized Living Aide to provide person-centered care for older adults in a community setting.

This regular part-time role includes daily living assistance, medication administration, housekeeping, and personal care under the guidance of the site nurse. You will collaborate with an interdisciplinary team, implement care plans, document activities, and promote client safety following program policies and regulatory

Qualifications

• High School Diploma or equivalent required.
• Prior experience working in a health care setting with older adults preferred.
• Certified Nursing Assistant, Home Health Aide Certificate or Trained Medication Assistant required.

Responsibilities

• Provide person-centered care to older adults and adults with chronic conditions.
• Assist with daily living activities including housekeeping, laundry, and personal care.
• Administer medications as instructed and participate in care planning.
• Communicate with the interdisciplinary team and document care in accordance with policy.

About the Role

Wilder's Customized Living follows a person-centered approach, valuing the uniqueness of each individual through our philosophy, policies, and procedures. Together with Wilder’s Healthy Aging & Caregiving Services, you will join a collaborative team that supports the health and independence of older adults as they age and receive care in the community, which is critical to Wilder’s mission of improving lives today and for generations to come. As a valued team member, you will provide person-centered care to older adults and adults with chronic conditions, supporting them with daily living assistance, including housekeeping, laundry, medication administration, and personal care. You will promote client safety, implement care plans, and provide basic rehabilitative services utilizing rehabilitative principles as instructed by the site nurse. Communicate regularly with an interdisciplinary team to support client care and complete documentation in accordance with program policies and procedures, regulatory requirements and best practice standards.

Qualifications

High School Diploma or equivalent required. Prior experience working in a health care setting with older adults and/or disabled adults preferred. Certified Nursing Assistant, Home Health Aide Certificate or Trained Medication Assistant required.

• 64 hrs/pay period; 3:00pm- 11:30pm; PM Shift
• 40 hrs/pay period; 3:00pm- 9:30pm; Evening Shift
